Studio City Rehabilitation Center is seeking a Social Services Assistant to join our team! Under the direct supervision of the Social Services Director the primary purpose of your job position is to assist in all aspects of the Social Services department as assigned and in accordance with current and existing Federal, State, and Local standards, as well as our established policies and procedures to ensure that the emotional and social needs of the resident are met and maintained on an individual basis.

Social Services Assistant Requirements

• Must have a valid California CNA license and CPR certification
• Proven work experience as a social worker
• BS degree in social work (BSW) preferred, psychology or sociology preferred, but not a requirement
• Working knowledge of social theories and practices
• Social perceptiveness and empathy
• Ability to build and maintain professional helping relationships
• Ability to relate and communicate with diverse population and groups
• Resilience along with ability to assess situations

Primary Social Services Assistant Responsibilities

• Assists in the planning, developing, organizing, implementing, evaluating, and directing of the social services programs of the facility
• Meet with administration, medical and nursing staff, and other related departments in planning social services
• Develop and maintain a good working rapport with inter-department personnel, other departments within the facility, and outside community health, welfare, and social agencies, to assure that social service programs can be properly maintained to meet the needs of the patient
• Assist in the development, administering, and coordinating of department policies and procedures
• Schedule transportation for SNF resident appointments and accompany only residents with no family. If resident has family, offer care giving services.
• PCS forms if needed in a timely manner
• Call PR of if the resident is self responsible then inform them for IDT meetings at least 2 weeks in advance.
• Manage ancillary services to ensure they are coming as needed
• Manage physician's medical order release forms
• Search for missing belongings as needed
• Label and place belongings on inventory list when families bring new belongings
• Bring packages to residents and open boxes with them to ensure facility protocols are followed
• Make sure annual inventory lists are done
• Assists with the admissions process; assists in the determination of admissions appropriateness, compile assessment information on incoming residents’ mental and physical condition in addition to financial status, and completes necessary forms
• Other duties may be assigned

PAY SCALE: $23 - $25 / hour

Depending on a number of factors, including, but not limited to location, regularly scheduled work shift, knowledge, skills, experience, and expertise.
